For the Hendersonville Wildcats Basketball League season(s) and any additional tournaments, I, as the participant (and/or legal guardian of the participant) stated above, consent to participate in all basketball and team related activities, and I certify that I am in good health and can take part in all activities. I fully understand that participation may involve serious risks and danger that may result in harm, bodily injury, and death. While rules and personal discipline may reduce the risk, I acknowledge that the risk of serious injury does exist. In the event of an emergency or an injury, I authorize the staff members to take all proper action and use the emergency service available at the nearest hospital if necessary. I understand that my personal insurance will be used in this case. I, for myself and on behalf of the participant hereby release and hold harmless against any claims, damages, and expenses, the directors, coaches, staff, officials, agents, and/or employees, other participants, and, if applicable, owners, lessors, and staff of the premises used to conduct the event. I acknowledge and agree that Mia Thysell and other board members of the Hendersonville Wildcats Basketball League retains the right to use photographs and videos taken of the participants for postings and announcements in print and social media.
